You made a character and leveled it up and had a set of actions to do and skillchecks to pass and could even meet other players (with limited interactions, you'd get their name and what they look like and get bonuses for visiting or dueling them)
The setting of that was that there is a huge mountain
Inside the mountain two races live, the humanoid 'mutants' and the 'abominations' which are all kinds of monsters
Mutants were humanoids that could look like anything mammal, reptilian, tentacly, a mix of the above, etc, basically evolving to be perfect hunters by whatever means (think of Monsters Inc but sticking to more humanoid appearances)
The stronger you got, the higher you could climb, literally, runts of the litter were at the caves in the bottom of the mountain while the strongest mutants were at the caves on the top
And of course the abominations were a constant threat and existed to be a threat to the mutants but also to be hunted by them for food
